A 66 year old man presents to you complaining he has been seeing double since yesterday afternoon. He denies head trauma.

On exam, he has unilateral left-sided ptosis, and is unable to move his eye upward, downward, or inward. Pupils are 6mm and unreactive on left, 3mm and reactive on right.
